Membership Forms
A membership kit containing all applications for new Single Adult or Family membership, code of ethics and sponsor's questionnaires as well as supporting documentation is available on the STCA Membership page.

Forms for Junior Membership or Membership Reapplication if membership lapsed within the dues year may be found there as well.

Those wishing to sponsor an HONORARY MEMBERSHIP NOMINATION may complete a sponsor form in pdf version which is available for download from the STCA Membership page.

Membership Renewal Dues Packet (PDF: 1.04MB / 11 pages)  

This packet contains all the necessary forms you will need to renew your STCA Membership in one packet. Download and print the pages you will need for your donation, membership renewal, awards nomination or application to serve the STCA.

STCA Income Voucher (PDF: 39KB / 1 page)  

This form is to be used to report all STCA income. When submitting money orders and checks, insure they are payable in US Funds and made payable to the STCA. Income submissions to the Rescue and Health Trusts must be sent to the respective fund treasurer and made payable to the RTF or HTF. Do not use this form for donations to the HTF or RTF.

STCA Expense Voucher (PDF: 36KB / 1 page)  

This form is to be used when requesting payment for authorized STCA expenses, original receipts must be included with the voucher.

STCA Credit Card Form (PDF: 158KB / 1 page)  

This form may be used to make payment using a VISA or MasterCard to the STCA for Membership, Bagpiper ads, Bagpiper Subscriptions, Ways and Means purchases, Dinner/Banquet/Lunch purchases, and any other payments made directly to the STCA. This form cannot be used for HTF donations, RTF donations, and Entry Fee payments to Show Super indents.

Health Trust Donation Form (PDF: 155KB / 1 page)  

This form may be used by any person who wishes to make a donation to the Health Trust Fund (HTF). The HTF is a 501c(3) corporation.

Rescue Trust Donation Form (PDF: 156KB / 1 page)  

This form may be used by any person who wishes to make a donation to the Rescue Trust Fund (RTF). The RTF is a 501c(3) corporation.

Health Trust Fund Trustee Application Form (PDF: 84KB / 1 page)  

This form is required to be completed by any STCA member in good standing who wishes to serve as a Trustee on the Health Trust Fund (HTF).

Rescue Trust Fund Trustee Application Form (PDF: 84KB / 1 page)  

This form is required to be completed by any STCA member in good standing who wishes to serve as a Trustee on the Rescue Trust Fund (RTF).

Specialty Judge Nomination Form (PDF: 18KB / 1 page)  

This form is to be used by any STCA member in good standing to suggest a judge for either the Rotating or National Specialty show and/or Sweepstakes.

Petition For Service as an Officer or Director (PDF: 63KB / 1 page)  

This form is to be used by any STCA member in good standing to indicate their desire to serve the STCA as an Officer or member of the Board of Directors.

Rescue Application (Online) 

Those individuals interested in adopting a Scottie Rescue dog may fill out our On-Line Application Form  and the information will be automatically submitted to the local Rescue Coordinator in your area.

STCA Breeder Referral Listing Request Form (PDF: 60KB / 1 page)  

This form is to be used when requesting that you be listed on the STCA Breeder Referral listing on line and printed listing.

STCA Dues Notice (PDF: 357KB / 1 page)  

This form is to be used to report all changes in your address, etc. in the STCA Membership Roster and to pay your membership dues for the current year.

Awards Nomination Form (PDF: 19KB / 1 page)  

This form is used to nominate an individual for any of the STCA Annual awards. Those awards include the Anstamm Achievement Award, the STCA Good Sportsmanship Award, and the Sterling Silver Medallion.
